Kosciusko County creates new Developmental Disabilities form

KOSCIUSKO COUNTY, Ind. – The Kosciusko County Sheriff’s Office has created a new Developmental Disabilities Registration Form to aid First Responders with potentially critical details in the event of an emergency.

This registration form is for any type of disability, including Autism Spectrum Disorders. The form was created with the help of The Bowen Center and Miller’s Merry Manor.

Those interested in registering can download the form found at the bottom of the Sheriff’s Office homepage under the title “Forms”. Instructions for returning the form are located in the final paragraph of the form.

Once the form is completed and submitted to the Sheriff’s Office communications center, it will be attached to readily accessible name and address records.

Users can also send a photograph of the individual alongside the registration form.
