South Bend health center reports potential data breach

SOUTH BEND, Ind. -- The South Bend location of Indiana Health Centers, Inc. says it discovered a breach of personal health care information on August 21.

The center says there was a break-in at the facility and the suspect or suspects took three computers that are believed to contain personal health care information.

Local law enforcement has been notified and affected patients will be notified.

As of now, the center says they have not been made aware of any misuse of patient information.

Because of the breach, patients may request a free copy of their credit report by visiting annualcreditreport.com, call 1-877-322-8228. Or complete the Annual Credit Report Request Form and mail it to: Annual Credit Report Request Service, P.O. Box 105281, Atlanta, GA 30348-5281.
