Indiana residents asked to report storm damage

Indiana residents who sustained damage in the storm on August 15/16 are being asked to fill out a damage assessment report.

It is important to fill out the form so the state can determine if the damage is extensive enough to request a disaster declaration from President Obama.

Damage reports will be used to help local emergency management agencies and the Indiana Department of Homeland Security with preliminary damage assessments and to determine if federal assistance – such as disaster grants or loans -- can be pursued.

However, it is not an application for assistance.

Individuals will be asked to provide their name, address, phone number and type of damage the property sustained. Losses can include structural damage to homes and loss of personal property.
