Statewide Silver Alert for 37-year-old man

WELLS COUNTY, Ind. - A Statewide Silver Alert has been declared for Jeffery Chesak, a 37-year-old man last seen in Ossian on June 9.
Chesak is described as a white male, 5’9” tall, 144 pounds, with brown hair and brown eyes.
He was last seen driving a dark blue 2014 Subaru Forrester with Indiana license plate BEF423.
Chesak was last seen at 8:30 p.m. on Wednesday, June 9. He is believed to be in extreme danger and may need medical assistance.
