Two dead after crash on S.R. 15

KOSCIUSKO COUNTY, Ind. – Two people have died and another person is injured after a crash involving three vehicles Monday morning, the Kosciusko County Sheriff’s Office reported.

At 10:43 a.m., emergency crews were called to a three-vehicle crash on S.R. 15, south of C.R. 1000 N.

77-year-old Ray Frost of Larwill was traveling north on S.R. 15 and as he was trying to pass a northbound vehicle in front of him, his vehicle hit a Pontiac traveling south, reports said.

Frost’s vehicle then continued north in the southbound lane and hit another vehicle driven by 51-year-old Mary Velazquez of Winona Lake.

Frost was partially ejected and pinned under his vehicle, which had rolled.

Velazquez was extricated from her vehicle by the Milford Fire Department.

Both Frost and Velazquez were pronounced dead at the scene.

The third driver was taken to a nearby hospital for evaluation.

This crash remains under investigation by the Kosciusko County Sheriff’s Office Fatal Team.
