IOSHA: No violations in fatal fire

The Indiana Department of Labor says an Indiana Occupational Safety and Health Administration investigation into the fatal K-Fex fire found no safety violations.
New Carlisle Assistant Fire Chief Jamie Middlebrook was killed August 5, 2014 while fighting a fire at K-Fex, Inc., an excavation and septic company on North 800 East in New Carlisle.
Middlebrook was killed when the roof of a barn on the property collapsed.
IOSHA investigated and found all training records were up to date and all involved were up to date on training.
IOSHA also found appropriate procedures were followed while fighting the fire.
No safety order will be issued.
