KOSCIUSKO COUNTY, Ind. -- The Kosciusko County Sheriff's Office reports deputies responded to a two-vehicle crash at the intersection of S.R. 15 and C.R. 200 S around 10:45 a.m. Tuesday.

Deputies arrived on scene to find a Nissan pick-up truck and a Freightliner box truck involved in the crash.

The initial investigation determined the Nissan, driven by 94-year-old Gene B. Butts of Warsaw, was traveling north on S.R. 15 while the box truck was traveling west on C.R. 200 S.

Authorities say the two vehicles collided at the intersection.

Butts was pronounced dead at the scene by the Kosciusko County Coroner.

The box truck driver and her two passengers were uninjured.

The crash remains under investigation by the KCSO FACT Team.
